Liposuction for Sculpting, Not Slimming | Dr Joni Feldman Melbourne

One of the biggest misconceptions I deal with in consultations is the idea that liposuction is a weight-loss tool. It isn’t — and honestly, it never was meant to be.

Most of my favourite results come from patients who are already at a healthy, stable weight. They eat well, they exercise regularly, and yet there’s still one area that just won’t shift no matter what they do. That’s genetics, not lifestyle. And that’s exactly where liposuction does its best work.

I think of these procedures less as “removing fat” and more as sculpting — refining lines that are already there, rather than reshaping the whole body.

The areas I see most often

These are the spots where small changes make a disproportionately big difference, particularly for people who are otherwise lean:

  • Chin and jawline — A spoonful of fat removed here can completely change how defined someone’s profile looks. This is one of the classic examples of “small amount, big difference.”
  • Knees — Genuinely one of my favourite areas to work on. It takes skill to blend the fat above and below the knee so the result looks natural, but when it’s done well, it transforms how trousers and dresses sit, and how the leg looks in general.
  • Ankles — Often overlooked, but slim ankles can make a real difference to how the whole lower leg appears, especially for patients who already have toned calves and thighs.
  • Mons pubis — This is the soft tissue that sits above the pubic bone, often related to hormonal fat deposition from puberty. For some women it causes discomfort under clothing or during exercise; for others it’s purely about how things sit under fitted clothing.
  • Side of breast / bra-line area — This is a really common one for women who are otherwise at a stable weight but have a persistent roll or fullness at the side of the bra. It can be uncomfortable as well as bothersome aesthetically, and even supportive bras don’t always solve it.
  • Arms — Particularly the upper arm, where some people carry a genetically stubborn layer of fat regardless of how much they train. This is an area where definition from exercise is often “hidden” underneath that layer.

Why this distinction matters

If someone comes to me hoping liposuction will help them lose significant weight, I’ll have an honest conversation with them — that’s not what this procedure is for, and using it that way can lead to disappointing results. Diet, exercise, and medical weight management are the right tools for that.

But once someone’s at a weight they’re happy with, and it’s one or two specific areas holding them back from feeling proportionate, that’s where liposuction can make a genuinely dramatic difference — even when the volume of fat removed is small.

What this looks like in practice

As with all my procedures, these are performed under local anaesthesia with sedation, in my accredited day surgery in South Yarra. Patients stay awake, which helps me reposition and check symmetry as I go.

Recovery is generally straightforward: compression garments for a couple of weeks, with bruising and swelling settling within that time (though final results can take a couple of months to fully show).
